LTC/SNF LPN

Job Description

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Long-Term Care (LTC) and Skilled Nursing Facility (SNF) team. The ideal candidate will have strong clinical skills and a passion for providing high-quality patient care.

Job Responsibilities

• Provide direct patient care to residents in accordance with established care plans.
• Administer medications and treatments as prescribed by physicians.
• Assist with daily living activities to ensure residents’ comfort and well-being.
• Monitor and document patients' vital signs and health status.
• Collaborate with interdisciplinary teams to improve patient outcomes.
• Participate in the orientation and education of new staff members.

Requirements

• Minimum of 2 years of nursing experience in LTC/SNF settings.
• BLS Certification required.
• Experience with electronic medical records (EMR) systems, specifically PCC.
• IV certification is required for all LPNs.

Shift Details

This position requires a 12-hour shift from 6:00 AM to 6:00 PM, with every other weekend and holiday availability.

Patient Care Focus

Care for residents with various diagnoses including:

• Congestive Heart Failure (CHF)
• Dementia
• Ostomies and Catheters
• Depression and Anxiety
• Diabetes Mellitus
• Hypothyroidism and Hyperlipidemia
• Hypertension
• Hemiparesis due to CVA
• Alzheimer's disease
• Chronic pain and constipation
• Muscle weakness and A-fib
• Gastroesophageal Reflux Disease (GERD) and edema

Additional Information

Orientation will be location-specific, focusing on the North and South units. The nurse-to-patient ratio is 1:17.
