Licensed Practical Nurse (LPN)

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team. The successful candidate will play a critical role in providing high-quality care to our patients.

Key Responsibilities

• Provide nursing care to patients, including assessing their needs and developing individual care plans.
• Administer medications and treatments as prescribed by the healthcare team.
• Assist patients with daily activities and personal care to ensure comfort and safety.
• Monitor and document patient progress, reporting any changes to the nursing staff.
• Collaborate with other healthcare professionals to enhance patient care.
• Participate in patient education and support healthcare initiatives.

Qualifications

• Current LPN licensure is required.
• IV Certification is required.
• Experience in geriatric care or working with patients with chronic conditions is preferred.
• Strong communication and interpersonal skills.
• Ability to work effectively in a team-oriented environment.
• Flexibility to work weekends and holidays every other rotation.

Shifts

• Shifts available in both 8-hour and 12-hour formats.
• Location-specific orientation will be provided, requiring 1-2 shifts.

Patient Demographics

The ideal candidate will be comfortable working with a diverse patient population, including individuals with:

• Dementia
• Diabetes
• Hypertension
• Chronic kidney disease
• Mobility challenges and ADL assistance needs

Additional Information

• No Charge Differential at participating facilities.
• Backouts and quits with less than 14 days' notice will incur a fee.
