1. Consent to Participate

Caregiver consents to participate in the CareCommunity platform, including the creation of a professional profile, uploading of credentials, and making such information available to licensed homecare agencies, subject to caregiver's express approval.

2. Authorization to Share Information

Caregiver authorizes CareConnect to share the information provided, including credentials, certifications, immunization and physical exam records, and other professional details with agencies, solely for employment and placement purposes.

3. HIPAA Authorization

Caregiver acknowledges and consents that certain personal health information may be shared in compliance with HIPAA, and CareConnect agrees to handle such information as required by applicable law and its Business Associate Agreement.

4. No Guarantee of Employment

Caregiver acknowledges that participation in CareCommunity does not guarantee employment or placement with any agency. CareConnect is not responsible for the employment decisions of agencies.

5. Release of Liability

Caregiver releases CareConnect from any liability arising out of participation in the CareCommunity platform, except where caused by CareConnect's gross negligence or willful misconduct.

6. Binding Arbitration

Any disputes arising under this Release shall be resolved exclusively by binding arbitration in accordance with the rules of the American Arbitration Association. The governing law shall be the Commonwealth of Massachusetts.

7. Acknowledgment

Caregiver acknowledges that they have read, understood, and agree to the terms of this Release.
