Job Summary

We are seeking a dedicated Licensed Practical Nurse (LPN) to provide exceptional patient care. The ideal candidate will work closely with nursing staff to implement care plans and support patients with various health conditions.

Responsibilities

• Monitor and assess patients' health needs, including vital signs and overall well-being.
• Administer medications and treatments as prescribed.
• Assist patients with activities of daily living (ADLs) as needed.
• Document and maintain accurate patient records using the PCC electronic medical record system.
• Communicate effectively with patients, families, and healthcare team members regarding patient care and health status.
• Participate in team meetings and contribute to care planning discussions.

Qualifications

• Valid LPN license in the state of practice.
• IV Certification required.
• Experience working in a long-term care or skilled nursing environment preferred.
• Strong communication and interpersonal skills.
• Ability to work effectively in a team-oriented and fast-paced environment.

Work Schedule

Shifts available are both 8-hour and 12-hour formats. Weekend and holiday coverage is required on an alternating basis. Position includes location-specific orientation for a duration of 1-2 shifts.

Patient Population

This position involves caring for patients with a variety of diagnoses, including dementia, diabetes, hypertension, chronic kidney disease, and mobility assistance needs related to falls or activities of daily living.
