LTC/SNF LPN

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Long-Term Care/Skilled Nursing Facility (LTC/SNF) team. The ideal candidate will have experience in providing high-quality care to residents and will be dedicated to meeting the needs of our diverse patient population.

Job Details

• Shift Length: 12 hours
• Guaranteed Hours: 36 hours per week
• Required Experience: 2 years
• Certification Required: BLS Certification
• Shifts: 6a-6p; every other weekend and holiday

Responsibilities

• Provide direct nursing care to residents in accordance with established standards and protocols.
• Monitor patients' health status and report changes to the supervising nurse.
• Administer medications and treatments as prescribed.
• Assist in the development and implementation of patient care plans.
• Maintain accurate and up-to-date patient records using electronic medical records (EMR) system.
• Support residents with activities of daily living and promote their independence.
• Communicate effectively with patients, families, and the healthcare team.

Qualifications

• Current LPN license in good standing.
• Experience in an LTC/SNF setting preferred.
• Knowledge of various medical conditions and treatments.
• Ability to work effectively in a team-oriented environment.
• Strong interpersonal and communication skills.

Additional Notes

• Local rates apply within 80 miles.
• Weekends and holidays required every other.
• Orientation provided specific to unit locations.
• Common patient diagnoses include CHF, Dementia, Ostomy, and Diabetes among others.
