Job Description

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team. The ideal candidate will work in a supportive environment, providing essential nursing care to patients with varying needs.

Key Responsibilities

• Provide nursing care, including administering medications and monitoring patient conditions.
• Assist patients with Activities of Daily Living (ADLs) and personal care.
• Collaborate with a multidisciplinary team to develop and implement patient care plans.
• Maintain accurate and timely documentation using electronic medical records (EMR).
• Engage with families to provide emotional support and education regarding care plans.

Qualifications

• Current LPN licensure in the applicable state.
• Basic Life Support (BLS) certification.
• Experience with IV therapy is required.
• Strong communication and interpersonal skills.
• Ability to work every other weekend and holiday.

Work Environment

The facility comprises two main units, with a total of 42 beds. The average nurse-to-patient ratio is 1:22, ensuring adequate support for nursing staff. The patient population includes those with chronic conditions such as dementia, diabetes, hypertension, and chronic kidney disease, as well as individuals requiring assistance with ADLs.

Orientation and Training

New hires will undergo location-specific orientation lasting 1-2 shifts to ensure a smooth transition into the role.
