LPN – Long-Term Care

Schedule

Shift: 2:00 pm - 10:00 pm (EOW required)

Assignment Details

Guaranteed Hours: 37.5 hours/week
Contract Length: 13 weeks

Requirements

• Active LPN license in the state
• 1+ years of experience in a long-term care facility
• Current BLS certification
• Skills checklist matching specialty
• Drivers license with address matching demographics
• NURSYS report for license verification
• Experience with PointClick Care EMR system

Responsibilities

• Provide direct patient care in compliance with established protocols
• Assist with assessments and documentation in patient records
• Administer medications and monitor patient responses
• Coordinate with interdisciplinary teams to implement care plans
• Educate patients and families on health conditions and treatment plans
