Job Description

We are seeking a dedicated Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to join our team in a Long-Term Care (LTC) facility. The selected candidate will work on a unit that focuses on various patient diagnoses, including Multiple Sclerosis, Orthopedic issues, Dementia, and Memory Care.

Unit Information

• Unit Type: Long-Term Care (LTC)
• Patient Beds: 40
• Common Patient Diagnoses: MS, Ortho, Dementia, Memory Care

Shift Details

• Shift Hours: 2:00 PM - 10:00 PM
• Guaranteed Hours: 37.5 hours per week
• Weekend Requirements: Every other weekend required
• Floating: Required within the same building
• Scrub Color: Navy Blue

Qualifications

• 1+ years of experience in a Long-Term Care (LTC) facility
• Current Basic Life Support (BLS) certification
• Active LPN or LVN license in NY and any other active licenses
• Skills checklist matching the specialty of the unit

Additional Information

Candidates must provide a current resume detailing their experience in an LTC facility, including any relevant travel experience. RTO (requested time off) should be included at the time of submission. Familiarity with the electronic medical record system PointClick Care is preferred.
