Job Description
The Home Care Registered Nurse is responsible for providing comprehensive nursing care to patients in their homes, ensuring high-quality and evidence-based practice. The ideal candidate will perform the following duties:
- Complete OASIS documentation in alignment with agency policies and federal regulations, maintaining current knowledge of documentation requirements.
- Assess patient and caregiver needs using the nursing process to facilitate the delivery of quality care.
- Develop and communicate realistic plans of care that involve patients and caregivers in the decision-making process.
- Act as Case Manager for assigned patients, effectively managing caseloads and visit schedules to meet agency productivity standards.
- Implement medical plans of care and nursing interventions, adhering to established safety and quality care standards.
- Stay updated on high-tech nursing skills including IV maintenance and complex wound care while ensuring infection control standards are met.
- Collaborate with Licensed Practical Nurses (LPNs) within their scope of practice, ensuring effective care delivery and task delegation.
- Evaluate the effectiveness of the plan of care by monitoring patients' responses and adjusting care as necessary.
- Maintain accurate and up-to-date documentation of nursing care to ensure seamless communication within the healthcare team.
- Follow agency protocols for documentation and visit procedures, ensuring visits are completed timely.
- Provide education to patients and caregivers to empower informed decision-making regarding care and treatment options.
- Identify interdisciplinary needs for patients and communicate them effectively to ensure comprehensive care.
- Complete required forms accurately and in a timely manner during patient visits.
- Conduct visits efficiently, ensuring supervisory visits are not combined with routine patient visits unless necessary.
- Assess patient needs for Home Care Aide (HCA) services and develop comprehensive care plans accordingly.
- Report patient incidents to quality improvement specialists as necessary.
- Coordinate safe patient discharges and transfers, ensuring all paperwork and patient information are processed correctly.
- Collaborate with the healthcare team, including physicians and vendors, to ensure safe and effective patient care.
- Participate in monthly peer review audits, providing and receiving constructive feedback professionally.
- Facilitate case conferences with team members to enhance communication and collaboration in patient care.
- Advocate for patient care issues, ensuring that the quality of care is sensitive to the specific needs of each patient and family.
