**Join Our Dynamic Home Care Team as a Registered Nurse!** **Job Description:** - Are you ready to make a significant impact on patients' lives in the comfort of their homes? Join our home care and hospice team, where your nursing skills can truly shine! - Work Schedule: Sunday - Thursday, 8am - 4pm, with additional on-call rotation. - Essential Responsibilities: - Complete OASIS documentation efficiently, adhering to agency policies and regulations. - Assess patient and caregiver needs using the nursing process to provide top-quality care. - Develop realistic care plans that actively involve patients and caregivers. - Serve as a Case Manager, managing your caseload effectively to meet productivity standards. - Implement medical plans governed by evidence-based practice and regulations. - Stay updated on advanced nursing skills such as IV therapy, wound care, and infection control. - Collaborate with LPNs to ensure effective care, delegate tasks, and review documentation. - Evaluate and adjust care plans based on patient needs and responses to treatment. - Maintain accurate and up-to-date documentation of nursing care. - Follow agency protocols for visit management and documentation processes. - Educate patients and caregivers about medication, treatment options, and care instructions. - Identify patients' interdisciplinary needs and communicate these needs promptly. - Complete agency, state, and federal forms accurately during visits. - Ensure time-efficient visits while communicating effectively with scheduling personnel. - Assess patients for HCA services and coordinate ongoing care. - Report patient incidents as required. - Coordinate safe discharges and transfers, ensuring proper documentation. - Collaborate with a multidisciplinary team to support patient care plans. - Participate in peer review audits and engage in constructive feedback. - Schedule and facilitate case conferences to enhance communication with team members. - Advocate for patient care issues, ensuring sensitivity to each patient's needs. --- **Why Catalytic Solutions (CatSol):** At CatSol, we connect dedicated therapists with rewarding assignments across the country.
