**Home Care Registered Nurse** Job Description: - Join a dynamic team where you impact the lives of patients and their families every day! - Provide compassionate hospice coverage when on-call, ensuring top-notch care for those in need. - Master the OASIS documentation process — stay informed and compliant while making a difference. - Use the nursing process to assess patient and caregiver needs, delivering quality care based on evidence-based practices. - Create and communicate personalized care plans that involve patients and their caregivers, ensuring they are part of the journey to recovery. - Serve as a Case Manager for your assigned patients, actively managing caseloads while meeting agency productivity standards. - Implement medical care plans and nursing interventions that maintain a high standard of quality and safety. - Enhance your skills with high-tech procedures like IV maintenance and complex wound care while adhering to infection control protocols. - Collaborate seamlessly with LPNs to delegate tasks and support the care plan, ensuring all patient needs are met. - Evaluate the effectiveness of care plans continuously, adapting as needed to ensure the highest quality of care. - Maintain clear and accurate documentation to support ongoing quality of care and effective communication among the healthcare team. - Navigate agency protocols and utilize McKesson processes for efficient visit management. - Educate patients and caregivers on crucial information such as medication side effects and treatment options. - Identify interdisciplinary needs and proactively communicate these to relevant team members. - Complete required agency, state, and federal forms accurately and on time. - Conduct visits with efficient time management, prioritizing supervisory visits as needed. - Assess patient needs for Home Care Aide services, coordinating plans for ongoing support. - Report patient incidents promptly to the Quality Improvement Specialist. - Oversee safe patient discharges and transfers, ensuring all necessary information is processed correctly. - Coordinate with physicians, providers, and team members to support comprehensive care plans. - Engage in monthly peer review audits, providing and accepting constructive feedback to foster professional growth. - Schedule and facilitate case conferences with managers and team members to enhance communication. - Advocate for patient care issues, ensuring sensitivity to the unique needs of each patient and family.
