**TITLE:** Licensed Vocational Nurse – Home Health **SCHEDULE:** 8:00 AM - 4:30 PM **ASSIGNMENT DETAILS:** - Contract Length: 13 weeks - Guaranteed Hours: 40.00 **REQUIREMENTS:** - Licensed Vocational Nurse (LVN) licensure in the state of practice: Required - Basic Life Support (BLS) Health Care Provider certification: Required - Arrhythmia certification: Preferred - Valid Driver’s License: Preferred - At least two years recent experience in the field **ROLES & RESPONSIBILITIES:** - Provide nursing care for assigned patients under the supervision of a Registered Nurse (RN) according to established standards and nursing processes. - Perform routine patient visits as dictated by departmental policies and procedures. - Assist in developing a prioritized interdisciplinary plan of care for each patient. - Implement and coordinate the plan of care utilizing organizational skills and appropriate reporting mechanisms. - Administer medications and perform assigned clinical procedures such as phlebotomy and injections. - Maintain cleanliness and organization in assigned areas, ordering and restocking supplies as necessary.
