LVN – Case Manager

Schedule

Shift: 5x8 (08:00 - 17:00) - Can work remotely from CA ONLY. Weekend requirement: Yes.

Assignment Details

Pending license: No; must be active at the time of submission.

Requirements

• CA LVN License
• 1 year of Case Management experience, with work comp experience required
• BLS certification
• Flu vaccine (no exemptions/no declinations)
• Experience with Epic
• Experience with Work Comp Case Management

Responsibilities

• Coordinate and manage care for patients in a case management setting.
• Assess patient needs and develop comprehensive care plans.
• Collaborate with healthcare teams to ensure efficient patient care.
• Monitor patient progress and adjust care plans as necessary.
• Document case management activities in compliance with protocols.
• Educate patients about their treatment options and care plans.
