Licensed Practical Nurse – Long-Term Care
**SCHEDULE**
Full Time, Days/Nights, Every Other Weekend Required
**ASSIGNMENT DETAILS**

**REQUIREMENTS**
- NY LPN Licensure
- BLS (AHA or RQI)
- 1 Year Experience as LPN
**ROLES & RESPONSIBILITIES**
- Provide direct and indirect skilled nursing care for assigned residents.
- Collaborate with nursing staff, physicians, and ancillary staff to ensure comprehensive care.
- Assess and collect data relevant to the resident's physical and psychosocial condition.
- Contribute to the care plan by incorporating the resident and family in the process.
- Maintain current knowledge and competency in nursing practice.
- Assist in the mentoring and orientation of new staff.
**COMPLIANCE / NOTES**
- Occupational exposure to blood borne pathogens is reasonably anticipated.
- Required tasks may involve contact with blood, body fluids, or tissues.
