LPN – Long-Term Care
**Schedule**
Part Time (2 shifts one week, 3 shifts the next week + Every Other Weekend/Holiday)
**Assignment Details**
Contract Length: 27 weeks
**Requirements**
- NY LPN Licensure
- BLS Certification (AHA or RQI)
- 1 year of experience as an LPN
**Roles & Responsibilities**
- Provide direct and indirect skilled nursing care to assigned residents.
- Collect and communicate data relevant to residents' physical and psychosocial condition.
- Collaborate with the nursing team and interdisciplinary team to create and revise care plans.
- Participate in mentoring and orientation of new staff members.
- Ensure adherence to safety protocols and maintain a safe environment for residents.
- Maintain current knowledge and competency in nursing practice through continued education.
