Title: LPN – Long Term Care
Schedule:
- Full-time evening shifts
- Every other weekend required
- Candidates must be available to stay through the full holiday season and start on a Tuesday or Thursday
Assignment Details:
- Contract Length: 27 weeks

Requirements:
- NY LPN Licensure
- BLS Certification (AHA or RQI required)
- 1 year of experience as an LPN in a long-term care facility
Roles & Responsibilities:
- Provide direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse.
- Collect and communicate data relevant to residents' physical and psychosocial conditions to the RN and interdisciplinary team.
- Contribute to the development of culturally relevant resident care plans involving residents, families, and healthcare teams.
- Maintain current knowledge and competency in nursing practice and participate in the orientation and mentoring of staff.
- Ensure customer satisfaction through collaboration with the resident, family, and interdisciplinary team.
Compliance / Notes:
- Occupational exposure to bloodborne pathogens is reasonably anticipated; required tasks may involve contact with blood, body fluids, or tissues.
