Responsibilities
- Provide direct patient care under the supervision of registered nurses, including hygiene, mobility assistance, feeding support, and daily living activities
- Administer prescribed medications and treatments per physician orders, with accurate documentation in the charting system
- Monitor patient status, vital signs, intake and output, and report changes promptly to the charge nurse
- Support rehabilitation goals through assistance with exercises, ADLs, and mobility activities as directed by therapists
- Maintain infection prevention, patient safety, and privacy standards; ensure accurate and complete documentation in the electronic health record
- Collaborate with interdisciplinary team to coordinate care plans and communicate patient needs effectively

Requirements
- Active Licensed Practical Nurse license in Vermont (or eligibility for VT licensure)
- Current CPR/BLS certification
- Relevant experience in long-term care or acute rehabilitation settings preferred
- Ability to work night shifts and adapt to a fast-paced clinical environment
- Strong documentation and communication skills; proficient with standard electronic health records
- Legal right to work in the United States
