Senior LIFE is an innovative home and community based Medicare and Medicaid funded program which provides all-inclusive healthcare services and support to seniors living in the community. Responsibilities include but not limited to: 

• Identifying medical needs for individual members, and setting care priorities accordingly
• Developing ongoing Plans of Care, in partnership with Senior LIFE interdisciplinary teams
• Interpreting, recording and acting on diagnostic results, and performing advanced nursing techniques and critical care procedures
• Identifying and assessing major medical problems, and communicating potential needs to a physician or specialist

Qualifications

Applicants should have at least one year working specifically with an elderly or aging population, hold a current RN license, and valid driver's license.
