**Requirements**
- Current state license as a Registered Nurse - Required
- Proof of current CPR certification - Required
- Must have reliable transportation and insurance - Required
- 1-2 years experience as a Registered Nurse in a clinical care setting - Required
- 2+ years Home health experience - Required
- Homecare Homebase Software Experience - Required
- OASIS Experience - Required
- CHPN certification - Preferred
- Bachelors of Science in Nursing - Preferred
**Roles & Responsibilities**
- Assess home health and/or hospice patients to identify their physical, psychosocial, and environmental needs.
- Develop, document, and evaluate the Plan of Care (POC) to ensure quality care.
- Educate patients and families regarding care, medications, and home safety practices.
- Provide clinical direction to support staff to ensure service quality.
- Monitor assigned cases for compliance with payer requirements.
- Perform on-call responsibilities as assigned.
