Registered Nurse (RN) - Home Health

**Title** Home Health RN – Nursing **Schedule** - Shift: 8-hour days - Schedule: 5 days/week **Assignment Details** - Patient Population: Adults / Geriatrics **Requirements** - Minimum 1 year skilled home health RN experience - OASIS experience required - Required certification: BCLS - EMR: OASIS required; Epic preferred - Must possess reliable personal transportation **Roles & Responsibilities** - Perform home health admission, recertification, resumption, and discharge - Conduct wound care and medication assessment/administration - Initiate and maintain IV access as needed - Perform phlebotomy and pain assessment/management - Ensure compliance with infection control standards **Compliance / Notes** - References: 2 required covering 1 year within the last 3 years - Pre-start modules (~16 hours average) - Confirm time-off restrictions with hiring manager - Free parking available - Scrubs/Attire: Any color scrubs or business casual (no blue denim) - COVID vaccination requirements should be confirmed with the hiring manager
