Registered Nurse – Case Manager

Schedule

Shift: 4x10 or 5x8 Days with varying shift times including 0700-1530, 0730-1600, 0800-1630, 0830-1700, 0700-1730, 0730-1800. Weekend work is required every other weekend or 4 weekend days in a 4-week schedule.

Assignment Details

Contract Length: 13 or 26 weeks. Guaranteed Hours: 72 hours per 2 weeks, with the possibility of being called off for one 8-hour shift per 2 weeks.

Requirements

• Active RN license in hand from the state.
• Basic Life Support (BLS) certification.
• Acute Care experience required.
• Epic EMR experience required.
• 1 year of nursing experience.
• ACM certification preferred.

Responsibilities

• Provide case management services to patients within the acute care setting.
• Coordinate patient care and facilitate transitions of care.
• Utilize clinical knowledge to assess patient needs and develop plans of care.
• Collaborate with multidisciplinary teams to ensure quality patient outcomes.
• Document and maintain accurate records using electronic health records.
