Registered Nurse – Case Manager

Schedule

4x10 or 5x8 Days. Shift times may vary between 0700-1530, 0730-1600, 0800-1630, 0830-1700, 0700-1730, and 0730-1800.

Assignment Details

Contract Length: 13 or 26 Weeks. Guaranteed Hours: 72 Hours per 2 weeks. Traveler can be called off for 1, 8-hour shift per 2 weeks.

Requirements

• Active AK RN License
• BLS Certification
• 1 Year of Acute Care Experience
• Epic Experience
• ACM Certification Preferred

Responsibilities

• Manage patient cases and coordinate care plans.
• Collaborate with healthcare teams to facilitate discharge planning.
• Conduct assessments and evaluations of patient needs.
• Provide education and resources to patients and families.
• Ensure compliance with regulatory requirements.

Compliance / Notes

Parking is free. The clinician must be open to floating within their scope to meet facility needs, and work every other weekend or 4 weekend days in a 4-week schedule. Only one time off request per 13-week assignment of 5 days or less is allowed, and no holiday time off is permitted for Auto Offer.
