Registered Nurse – Case Manager

Schedule

Shift: 4x10 or 5x8 Days. Available shift times may vary, with options including 0700-1530, 0730-1600, 0800-1630, 0830-1700, 0700-1730, and 0730-1800. Weekend shifts are required (every other weekend or 4 weekend days in a 4-week schedule).

Assignment Details

Contract Length: 13 or 26 Weeks. Guaranteed Hours: 72 Hours per 2 weeks, with the possibility of being called off for 1, 8-hour shift per 2 weeks.

Requirements

  • Active RN license in the state required.
  • BLS certification required; ACM certification preferred.
  • Minimum of 1 year of experience required.
  • Epic experience required.
  • Acute Care experience required.
  • Must be located at least 50 miles from the facility to qualify.

Responsibilities

  • Coordinate patient care and manage case loads effectively.
  • Collaborate with interdisciplinary teams to develop comprehensive care plans.
  • Assess patient needs and resources, and provide appropriate interventions.
  • Educate patients and families on care plans and health management.
  • Document all patient interactions and progress in the EMR system.
  • Participate in quality improvement initiatives as needed.
