Registered Nurse – Case Manager

Schedule

Shift options are 4x10 or 5x8 days. Shift times may vary between 0700-1530, 0730-1600, 0800-1630, 0830-1700, 0700-1730, and 0730-1800. Weekend work is required every other weekend or 4 weekend days in a 4-week schedule.

Assignment Details

Contract length is available for either 13 or 26 weeks. Guaranteed hours are 72 hours per 2 weeks, with the potential for being called off for one 8-hour shift per 2 weeks.

Requirements

• A current RN license in the state is required.
• Minimum of 1 year of acute care experience is required.
• BLS certification is required; ACM certification is preferred.
• Must have Epic experience.

Responsibilities

• Coordinate patient care and collaborate with healthcare teams.
• Assess patient needs and develop appropriate care plans.
• Monitor patient outcomes and adjust care plans as necessary.
• Educate patients and their families on treatment options and resources.
• Facilitate communication among interdisciplinary teams to optimize patient care.

Compliance / Notes

Parking is provided free of charge. Only one time off request of 5 days or less is allowed per 13-week assignment, with no holiday RTO for auto offer. Candidates must reside at least 50 miles away from the facility.
