Assignment Details

Guaranteed hours: 70 hours per 2 weeks.

Requirements

• Active RN license in the state required.
• BLS certification required.
• ACM certification preferred.
• 1 year of acute care experience required.
• Epic experience required.
• Must be at least 50 miles from the facility to be eligible.

Responsibilities

• Manage patient care coordination and transitions.
• Assist with resource management and discharge planning.
• Collaborate with healthcare team members to optimize patient outcomes.
• Educate patients and families regarding care plans and healthcare resources.

Compliance / Notes

Weekend work required: every other weekend or a total of 4 weekend days in a 4-week schedule. Only 1 time-off request of 5 days or less is allowed per 13-week assignment, with no holiday time off for auto offers.
