**Join Our Team as a Registered Nurse in Home Care!** **Job Description:** - Be a vital part of our interdisciplinary care team providing forefront services to older individuals. - Work independently while applying advanced nursing knowledge and managing complex clinical situations. - Collaborate with both licensed and non-licensed personnel to enhance participant care. **Primary Responsibilities:** - Triage emergent situations effectively, ensuring high-quality participant care. - Monitor both acute and chronic medical conditions in coordination with Primary Care Providers (PCPs). - Utilize care plans independently to deliver clinic and home-based direct care. - Administer treatments, procedures, and medications as prescribed to participants. **Duties and Responsibilities:** - Regularly monitor participants’ acute and chronic medical conditions, aligning with their goals of care. - Perform wound care and treatments as prescribed in each participant's plan of care. - Administer medications, immunizations, and other care as needed. - Conduct assessments and actively engage in creating care plans. - Make regular home visits to fulfill care and treatment orders from PCPs, including wound care and lab draws. - Respond to on-call needs, determining immediate requirements for participants. - Be flexible with working hours, including occasional weekends and holidays, as necessary. - Complete required education and documentation on time. - Keep all parties informed about participant care issues and collaborate using a participant-centered care model. - Document in participants' medical records according to policy. - Take on additional duties as assigned, contributing to the team's success. **Qualifications:** - Bachelors of Science in Nursing preferred; Associate degree required. - RN Nursing Diploma or Degree from an accredited School of Nursing. - Proficient in Microsoft Word and Outlook. - Strong communication skills, both written and verbal. - Experience working with the frail, elderly, or long-term care population (minimum of one year preferred). - Home care experience is a plus, as well as wound care experience. **Specific Requirements:** - Current CPR certification by AHA or willingness to obtain. - Demonstrate compassion, integrity, teamwork, and excellence in your work. - Ability to efficiently use time and resources to provide exceptional customer service. - Independent decision-making abilities as circumstances require. - Familiarity with Senior Care Partners P.A.C.E. goals, objectives, policies, and procedures. - Maintain confidentiality regarding sensitive information. - Ensure immunizations and vaccines are current, or provide medical documentation for waiver requests. - Reliable transportation required with a valid Michigan driver’s license and vehicle insurance. **Work Environment:** - Active presence in the care center with frequent interruptions. - Communicate regularly with staff, participants, caregivers, and external organizations. - Potential exposure to infectious materials and hazardous conditions. - Availability for callbacks during emergencies and ability to drive in all weather conditions. **Physical Demands:** - Handle mental and emotional stresses; work independently with integrity. - Flexibility in working hours and the ability to push, pull, move, sit, or stand for extended periods. - Capability to lift up to twenty pounds when necessary. **Minimum Working Requirements:** - Must be at least 18 years of age. - Willing to be fully vaccinated against communicable diseases (Influenza, MMR, Varicella, Tdap/Td, Covid-19, and Hepatitis B). **Why Catalytic Solutions (CatSol):** At CatSol, we connect dedicated therapists with rewarding assignments across the country.
