About the Role:

We are seeking a compassionate and experienced Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our team at CHE Behavioral. As a PMHNP, you will play a critical role in providing high-quality behavioral health services to our patients in long-term care and rehabilitation centers. With a focus on improving mental health outcomes, you will work collaboratively with our multidisciplinary team to deliver patient-centered care that addresses the physical and emotional needs of our patients.

Responsibilities:

• Assess, diagnose, treat, and manage mental health conditions for patients in long-term care and rehabilitation centers

• Develop and implement individualized treatment plans tailored to each patient's unique needs

• Provide education and support to patients and their families on mental health conditions, treatments, and coping strategies

• Collaborate with our interdisciplinary team to ensure comprehensive care and coordination of services

• Conduct regular mental health evaluations, assessments, and re-evaluations as needed

• Stay up-to-date with the latest best practices and research in psychiatric-mental health nursing to inform clinical decision-making
