As a Psychiatric Mental Health Nurse Practitioner (PMHNP), you will play a vital role in providing high-quality mental health services to our patients in Albany, New York.

About the Role:

As a PMHNP, you will work collaboratively with a team of experienced mental health professionals to deliver comprehensive behavioral health services to older adult patients in long-term care and rehabilitation centers. You will diagnose, treat, and manage mental health conditions, including depression, anxiety, and substance abuse, using evidence-based practices and psychopharmacological interventions. PMHNP responsibilities will include:

• Conducting initial assessments and ongoing evaluations of patients' mental health status
• Developing and implementing individualized treatment plans tailored to each patient's needs
• Prescribing and managing medications as necessary
• Providing psychotherapy and counseling services to patients and their families
• Collaborating with other healthcare professionals to ensure seamless care coordination
• Documenting patient progress and outcomes in accordance with regulatory requirements
