Nurse Practitioner

Under the direction of the Chief Medical Officer, the Site Nurse Practitioner is responsible for the professional, organizational, and administrative aspects of providing quality medical care to Diagnostic Treatment Centers (DTCs) patients. Resumes with Geriatric, Primary Care, and Internal Medicine experience will be considered. Please see attached JD. DEA License, and Medicare & Medicaid IDs are a must, Homecare experience preferred or willing to do homecare Schedule: Monday - Friday 8:00am-5:00pm (1 hour lunch) Contract Length: Potential for temp to perm based on performance, business needs. The Nurse Practitioner (NP) will work in collaboration with the Organization’s primary care physicians to ensure proper oversight and coordination of patient care. The NP’s core responsibilities entail: gathering detailed histories, performing physical exam and conducting relevant diagnostic tests, diagnosing and treating common medical conditions within their scope of practice, managing patients with chronic illnesses and tailoring treatment plans accordingly, educating patients and their families about disease prevention, health promotion, and slow progression of chronic illnesses, referring to specialists when medically warranted, documenting accurate and complete patient records in the Electronic Health Records (Athena), adherence to professional standard professional conduct, participating in quality improvement initiatives to enhance patient care and outcomes, and work effectively within a healthcare team including healthcare professional, colleagues, patients and their families.
