### Title
LPN – Geriatric Nursing
### Schedule
Full-time nights, every other weekend and holiday required.
### Assignment Details
Contract Length: 27 weeks
### Requirements
- NY LPN Licensure
- BLS (AHA or RQI only)
- 1 year of experience as an LPN
### Roles & Responsibilities
- Provide direct and indirect skilled nursing care for assigned residents.
- Collect and communicate data regarding residents' physical and psychosocial conditions to the RN and interdisciplinary team.
- Contribute to the development of age-appropriate and culturally relevant care plans.
- Recommend revisions to care plans based on resident outcomes and collaborate with the interdisciplinary team.
- Maintain knowledge and competency in nursing practice and participate in mentoring or orientation of staff.
- Ensure compliance with safety and care standards while delivering nursing services.
