Licensed Practical Nurse (LPN)

Job Overview

We are seeking a dedicated and skilled Licensed Practical Nurse (LPN) to join our team in a long-term care and rehabilitation setting. The ideal candidate will have a strong commitment to providing quality patient care while working in a dynamic and supportive environment.

Responsibilities

• Provide nursing care and support to residents in accordance with their individual care plans.
• Administer medications and treatments as prescribed by healthcare providers.
• Assist with daily living activities and promote residents' independence.
• Monitor residents' health status and report any changes to the healthcare team.
• Maintain accurate and timely documentation using Point Click Care (PCC).
• Collaborate with interdisciplinary teams to ensure comprehensive care for residents.

Qualifications

• Minimum of 2 years of long-term care experience.
• Proficient in using Point Click Care for charting and documentation.
• Strong communication and interpersonal skills.
• Ability to work effectively in a team-oriented environment.
• Licensed Practical Nurse (LPN) license in good standing.

Work Schedule & Expectations

• Full-time opportunity with a shift length of 8 hours.
• Travelers must be flexible with scheduling, including working 2 out of 3 major holidays.
• Orientation is held every two weeks, with specific start dates available.
• RTO (Request Time Off) is not permitted prior to an offer being made.
