Registered Nurse (RN) – Case Manager
Irving, TX

Overview:
Our hospice care team in Irving is seeking a Registered Nurse (RN) Case Manager to provide comprehensive, compassionate care across facility and home settings. This full-time role offers a consistent Monday–Friday schedule within a collaborative interdisciplinary team.

Key Responsibilities:

• Conduct comprehensive patient assessments and full-body evaluations in facility and home settings
• Manage medication refills and coordinate symptom management needs
• Participate in Interdisciplinary Group (IDG) meetings and contribute to care coordination decisions
• Supervise and oversee CNAs during patient visits
• Collaborate with the interdisciplinary hospice team to ensure continuity of care
• Support occasional admissions rotation (minimal, as most admissions and GIP cases are handled by Clinical Liaisons)

Qualifications:

• Active Registered Nurse (RN) licensure in Texas
• Minimum 1 year of hospice nursing experience
• Valid driver's license and reliable transportation required
