Job Description
- RN Hospice Case Manager
- Performs comprehensive nursing evaluations.
- Assesses for signs and symptoms of discomfort including presence of pain and takes prompt and appropriate action.
- Initiates and manages plan of care, provides appropriate nursing follow –up to all patients, and instructs caregivers in managing patient needs.
- Coordinates all services provided to patients and families, as well as appropriateness of care, to ensure quality of care and services.
Job Requirements
- Education: Graduate of an accredited school of nursing.
- Credentials: Current RN License within CA. A current CPR Certification, Valid Driver’s License and auto insurance.
- Experience: Preferred one year experience as a professional nurse within the last 3 years with strong clinical assessment skills.
- Skills: All nursing functions, written and oral communication skills, sympathetic attitude, customer services/relations, ability to work with dying patients, organizational and diplomacy.
- Supervises: Provides supervision of Home Health Aides, Licenses Vocational Nurses, Licensed Practical Nurses, and Homemakers to ensure the best quality of care for patients.
