Job Summary
We are seeking a compassionate Hospice Registered Nurse (RN) to provide exceptional end-of-life care for patients and families in their homes and communities. The RN will perform comprehensive patient assessments, develop individualized care plans, coordinate care with the interdisciplinary team, and support patients through hospice and palliative care services.
Key Responsibilities
Conduct comprehensive patient and family assessments to determine hospice care needs.
Develop, implement, and update individualized care plans.
Provide skilled nursing care, symptom management, medication administration, and patient education.
Coordinate care with physicians, caregivers, and the interdisciplinary hospice team.
Serve as the primary Hospice RN for an assigned patient caseload.
Document patient conditions, interventions, and outcomes accurately and timely.
Educate patients and caregivers on disease management, medications, comfort measures, and self-care techniques.
Ensure continuity of care and advocate for patient comfort, dignity, and quality of life.
Participate in interdisciplinary team meetings and on-call rotation as needed.
Supervise hospice aides and support staff in accordance with agency policies.
Participate in quality improvement initiatives and continuing education programs.
Qualifications
Current, unrestricted Registered Nurse (RN) license in California.
Nursing experience; hospice, home health, palliative care, or public health experience preferred.
Current CPR certification.
Strong clinical assessment, critical thinking, communication, and care coordination skills.
Ability to work independently and manage a patient caseload effectively.
Valid driver's license, reliable transportation, and current auto insurance.
