Registered Nurse (RN) – Home Hospice & Palliative Care Case Manager – Full-Time Days

As a Home Hospice & Palliative Care RN Case Manager with ProHealth Care, you’ll provide compassionate, individualized nursing care to patients managing serious or life-limiting illness. You’ll work with patients and families in the home, helping manage symptoms, coordinate care and provide education and support throughout their journey.

  • Monday–Friday | 8:00 a.m.–4:30 p.m.
  • No regularly scheduled weekends
  • Two holidays per year one major and one minor
  • Back-up on-call rotation approximately four times per month
  • Occasional additional on-call coverage for night RN PTO
  • $10,000 sign-on bonus with a two-year commitment
  • Independent, relationship-focused nursing with the support of an interdisciplinary Hospice team

 

What you’ll do

  • Manage an assigned caseload of Hospice and Palliative Care patients
  • Complete comprehensive nursing assessments and contribute to individualized plans of care
  • Provide skilled nursing care, symptom management and ongoing assessment in the patient’s home
  • Educate patients and families about medications, symptom management, disease progression and what to expect
  • Coordinate care with physicians, hospice providers and the interdisciplinary care team
  • Serve as a consistent clinical resource for patients and families as their needs change
  • Identify changes in condition and collaborate with the care team to adjust the plan of care
  • Support patients and families through complex medical and end-of-life needs with compassion and respect
  • Complete timely clinical documentation and care coordination
  • Participate in the department’s back-up on-call and holiday rotation

 

What you’ll need

  • Current Registered Nurse license or eligibility for licensure in Wisconsin
  • Strong clinical assessment, critical-thinking and care coordination skills
  • Ability to work independently in patients’ homes while remaining connected to an interdisciplinary team
  • Strong communication skills and comfort providing education and support to patients and families
  • Ability to manage a caseload, prioritize changing patient needs and complete timely documentation
  • Valid driver’s license and ability to travel to patients throughout the service area
  • Hospice, palliative care, home health, case management or other relevant nursing experience is valued
