Registered Nurse (RN) - Home Health

**Title:** Registered Nurse – Home Health **Schedule:** 8 hours **Assignment Details:** - Contract Length: 13 weeks - Guaranteed Hours: 40 **Requirements:** - Current state license as a Registered Nurse - Homecare Homebase Software Experience - OASIS Experience - Proof of current CPR certification - Current driver's license - Must have reliable transportation and insurance - 1-2 years of experience as a Registered Nurse in a clinical care setting - 2+ years of Home health experience - CHPN certification (preferred) **Roles & Responsibilities:** - Assess and respond to the needs of patients and families in home health and hospice settings. - Develop, document, and evaluate individualized Plans of Care (POC) to ensure quality and continuity of care. - Educate patients and their families regarding care, medications, safety, and self-care practices. - Supervise and provide clinical direction to home health aides to ensure quality service delivery. - Monitor patient pain and implement appropriate pain management strategies. - Communicate effectively with patients, families, and the interdisciplinary team.
