**Title** Registered Nurse – Home Health **Schedule** 8 hours **Assignment Details** Contract Length: 13 weeks Guaranteed Hours: 40 **Requirements** - Current state license as a Registered Nurse - Homecare Homebase Software Experience - OASIS Experience - Proof of current CPR certification - Current driver's license - Reliable transportation and insurance - 1-2 years of experience as a Registered Nurse in a clinical care setting - 2+ years of Home health experience - CHPN certification (Preferred) **Roles & Responsibilities** - Assess patients in their home environment to identify their physical, psychosocial, and environmental needs. - Develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care. - Educate patients and families about care plans, medications, and safety practices. - Supervise and provide clinical directions to home health aides or support staff. - Monitor compliance with payer requirements and perform on-call responsibilities as needed.
