What You'll Do
As a Home Health RN, you will provide skilled nursing care to patients in their homes while working closely with physicians, patients, families, and the interdisciplinary care team.
Your responsibilities will include:
- Conducting comprehensive patient assessments and reassessments
- Developing, implementing, and updating individualized Plans of Care
- Providing skilled nursing interventions based on each patient's clinical needs
- Completing accurate and timely OASIS assessments and clinical documentation
- Educating patients and families regarding disease processes, medications, treatments, and plan of care
- Monitoring patient progress and communicating changes in condition appropriately
- Collaborating with physicians and the interdisciplinary care team
- Promoting patient safety, quality outcomes, and continuity of care
- Providing compassionate, professional care that allows patients to remain safely at home

Requirements
- Graduate of an accredited Registered Nursing program
- Current, unrestricted RN license in the state of Georgia
- Previous Home Health and OASIS experience required
- Current CPR certification
- Valid driver's license
- Reliable transportation and current automobile insurance
- Strong clinical assessment and critical-thinking skills
- Excellent communication and documentation skills
- Strong organization and time-management abilities
- Ability to work independently while remaining connected to the clinical team
- Dependable, professional, and patient-focused
