We provide evidence-based, holistic care through a full continuum of services, from residential treatment to outpatient services and recovery support. We are seeking a compassionate and dependable Registered Nurse (RN) to join our Medical Department on a PRN (as-needed) basis. This role requires flexibility to cover various shifts based on staffing needs.
As part of our medical team, the RN assesses client needs, provides nursing care, coordinates treatment, and educates clients on healthy habits that support long-term recovery. The ideal candidate understands the unique needs of individuals receiving substance use disorder and behavioral health treatment and is committed to delivering respectful, high-quality, client-centered care. We provide evidence-based, holistic care through a full continuum of services, from residential treatment to outpatient services and recovery support.
The RN supports the health, safety, and recovery of clients through nursing assessments, monitoring, education, and care coordination. Working closely with medical providers and the clinical team, the RN uses sound clinical judgment to identify concerns, respond to client needs, and support continuity of care throughout treatment.
Job Duties Include but are not limited to the following:
Client Care and Assessment
- Complete medical intake questionnaires, review medical histories, check vital signs, and assess clients for medical concerns and treatment needs.
- Collaborate with licensed providers to develop, implement, and evaluate treatment plans.
- Monitor clients reporting illness, document symptoms, and communicate findings to providers for guidance.
- Educate clients about medical conditions, health maintenance, and medication use.
- Collaborate with therapists to complete ASAM assessments.
- Recognize potential medical emergencies and respond promptly according to established protocols, including drug and alcohol overdose procedures.
- Notify emergency contacts during crisis situations, as appropriate, and document communications.
Medication and Treatment Support
- Coordinate medication orders under the direction of licensed providers and assist clients with self-administration of medications.
- Track medication orders for clients receiving Medication-Assisted Treatment (MAT).
- Complete required documentation for specialty pharmacy medications.
- Administer specialty injections and support medical procedures within the RN’s scope of practice.
- Conduct medication cabinet audits and follow established medication management procedures.
Care Coordination and Documentation
- Communicate with healthcare providers regarding client needs and treatment plan adjustments.
- Coordinate medical, dental, and specialty appointments with the logistics team.
- Schedule APRN appointments using eClinicalWorks (ECW) and prepare client information before appointments.
- Request outside medical records through appropriate channels.
- Review signed Releases of Information (ROIs) before communicating with family members or external providers.
- Maintain accurate, timely medical records in ECW, KIPU, and other required systems.
- Complete chart audits, monthly audits, and required reporting, including KHIE updates for reportable disease cases, as applicable.
Medical Operations and Team Support
- Keep medical rooms clean, organized, and adequately stocked, including first aid supplies.
- Contribute to the development and implementation of medical policies and procedures.
- Follow Isaiah House policies, treatment room protocols, and applicable nursing standards.
- Complete required training and competencies.
- Participate in the after-hours, on-call rotation as needed.
- Perform other duties as assigned within the RN’s scope of practice.
Key Job Responsibilities:
- Complete client medical evaluations, assessments, and treatment plan support.
- Monitor client health, vital signs, symptoms, and medication needs.
- Assist with medication ordering, administration support, MAT tracking, and injections.
- Maintain accurate EHR documentation, ASAMs, chart audits, and medical records.
- Coordinate care with providers, therapists, logistics, families, and emergency contacts.

RequirementsPhysical Requirements:
- Perform the essential functions of the position with or without reasonable accommodation.
- Stand, walk, and move between work areas for extended periods while providing care in a clinical setting.
- Lift, carry, push, or pull objects weighing up to 25–50 pounds.
- Perform clinical tasks requiring manual dexterity and coordination.
- Communicate effectively with clients and the care team, and observe and assess changes in client condition.
Key Performance Indicators (KPIs)
- Complete the Isaiah House new client intake process within 24 hours of intake.
- Submit medication orders before the pharmacy’s cutoff time.
- Maintain client confidentiality and comply with HIPAA and applicable SAMHSA confidentiality requirements at all times.
- Promptly report training, process, or human resources concerns to the RN Lead.
- Report client care concerns directly to the Lead APRN.
- Read and respond to work-related emails, Microsoft Teams messages, and texts in a timely manner during assigned shifts or on-call coverage.
- Attend assigned meetings on time and prepared to participate.
- Actively support the implementation of departmental policies and procedures.

- Maintain a respectful, professional attitude when interacting with clients, staff, volunteers, and students.
Required Qualifications:
- Minimum of an Associate Degree in Nursing.
- Current, valid, and unrestricted Registered Nurse license in the State of Kentucky.
- CPR certification prior to hire.
- Valid Kentucky driver’s license.
- Knowledge of emergency procedures related to alcohol and/or drug overdose.
- Ability to adhere to Isaiah House policies and procedures.
Preferred Qualifications:
- Bachelor’s Degree in Nursing preferred.
- Knowledge of recovery, mental health, and co-occurring disorders preferred.
- Experience working with addiction in a residential and/or intensive outpatient setting preferred.
- Experience in crisis intervention preferred.
- Conflict resolution skills preferred.
