Job Description:

Job Summary


Responsible for providing compassionate, patient-centered nursing care to individuals and families receiving hospice services. Delivers holistic care focused on comfort, dignity and quality of life through assessment, planning, implementation and evaluation of patient needs. Collaborates with physicians, interdisciplinary teams and caregivers to ensure seamless coordination of end-of-life care. Supports patients and families with empathy, education and emotional guidance throughout the hospice journey.


Schedule: Monday - Friday 8:00am-4:30pm

This position will also include an On-Call weekend rotation.


Essential Functions


• Provides direct nursing care to hospice patients in home or facility settings according to care plans.


• Assesses, monitors and documents patient symptoms, comfort levels and response to interventions.


• Administers medications and treatments to manage pain and other end-of-life symptoms.


• Collaborates with physicians and interdisciplinary team members to develop and update individualized care plans.


• Educates patients, families and caregivers on symptom management and end-of-life care practices.


• Supports emotional, psychosocial and spiritual needs of patients and families.


• Ensures adherence to hospice regulations, clinical policies and infection control standards.


• Coordinates care transitions and communicates effectively with care teams and external partners.


• Participates in case conferences, quality improvement activities and professional development.


• Performs other duties as assigned.




Education Requirement


Associate’s degree in nursing


Bachelor’s degree in nursing is preferred




Experience Requirement


0-2 years of experience in in nursing, or a relevant area




An equivalent combination of education and experience may be considered. All experience must be paid and in the same related field. Part-time and PRN experience will be prorated based on hours worked per week. Volunteer work and internships for academic credit are not counted.

Physical Demands
