Job Overview
This is an opportunity for a dedicated Registered Nurse Case Manager to join our client and make a difference in the lives of patients in hospice care. You will be part of a supportive team, providing essential care and coordination for patients and their families. Key Responsibilities ● Manage patient care in hospice settings ● Coordinate with healthcare teams ● Provide leadership and support to nursing staff Schedule and Shift Details Schedule: Monday – Friday 8:00 am – 6:00 pm, 1 weekend per month, and 2 holidays per year Travel ● Local travel required covering multiple counties.

Requirements

Qualifications

• High School/GED
  

• Hospice (1+ years)
  

• Management (1+ years)
  

• 1+ years of experience in ANY of the following:
  

  • Home Health Care
    

  • Epic Home Health
    

  • Home Health
    

• Valid Registered Nurse (RN) in the state of Pennsylvania (PA)
  

• ALL of the following valid licenses/certifications:
  

  • American Heart Association Basic Life Support (AHA BLS)
    

  • American Heart Association Advanced Cardiac Life Support (AHA ACLS)
    

• Have you applied to or worked for Residential Healthcare Group anytime in the last 6 months? (no)
  

• Are you okay with the required travel for this position? (yes)
