Position Summary
Rocky Mountain Care is seeking a compassionate and skilled Registered Nurse (RN) to join our Home Health team. The Home Health RN provides high-quality nursing care to patients in their homes, helping them maintain independence, manage chronic conditions, recover from illness or surgery, and improve overall health outcomes. The ideal candidate is patient-focused, organized, and committed to delivering exceptional care while collaborating with physicians, caregivers, and interdisciplinary healthcare teams.
Key Responsibilities
- Perform comprehensive patient assessments and develop individualized care plans.
- Provide skilled nursing care in the home setting, including wound care, medication management, disease management, and patient education.
- Monitor patient progress and update care plans as needed.
- Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
- Educate patients and family members on treatment plans, medications, disease processes, and self-care techniques.
- Complete timely and accurate documentation in the electronic medical record (EMR).
- Assess home environments to identify safety concerns and recommend appropriate interventions.
- Respond to changes in patient conditions and communicate effectively with the care team.
- Ensure compliance with federal, state, and organizational regulations and standards.
- Participate in case conferences and team meetings to support positive patient outcomes.
Qualifications
Required Qualifications
- Current and unrestricted Registered Nurse (RN) license in the State of Utah.
- Graduate of an accredited nursing program.
- Minimum of one (1) year of nursing experience required.
- Current BLS (Basic Life Support) certification.
- Valid driver's license, reliable transportation, and proof of auto insurance.
- Ability to travel throughout assigned service areas.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently while managing multiple patient visits and priorities.
Preferred Qualifications
- Previous Home Health, Hospice, Skilled Nursing, or Case Management experience.
- OASIS documentation experience.
- Experience with Medicare and Medicaid home health regulations.
- IV therapy and wound care experience.
Physical Requirements
- Ability to lift, transfer, or assist patients as necessary.
- Frequent standing, walking, bending, and driving between patient homes.
- Ability to safely navigate various home environments.
- Ability to utilize standard nursing and medical equipment.
