Licensed Practical Nurse (LPN) - Per Diem Shifts

Alvita Care is seeking compassionate, reliable, and professional Licensed Practical Nurses (LPNs) for immediate full-time floor staffing opportunities in Patchogue, NY.

Available Shifts

• Primary Shifts: Most available shifts are 3 PM–11 PM, on weekdays and weekends.
• Employment Options: Full-time or part-time hours available.
• Per-Diem Positions: All positions are per diem, with scheduling based on facility staffing needs.

POSITION DETAILS

• Position: Licensed Practical Nurse (LPN)
• Employment Type: Full-Time
• Location: Patchogue, Suffolk County, NY
• Setting: Facility Floor Staffing
• Pay Rate: $40–$45/hour, based on experience and assignment
• Availability: Full-time schedule based on facility staffing needs

RESPONSIBILITIES

• Provide quality nursing care and support to residents.
• Administer medications and treatments within the scope of the LPN license.
• Monitor residents' conditions and report changes to the appropriate healthcare professionals.
• Maintain accurate and timely documentation using the facility's charting system.
• Follow facility policies, procedures, and infection control protocols.
• Collaborate with other healthcare professionals to promote resident safety and well-being.
• Assist with other nursing duties as assigned within the scope of practice.

QUALIFICATIONS

• Valid New York State LPN license required.
• Previous nursing experience preferred.
• Long-term care or facility experience preferred.
• Reliable, professional, and patient-centered.
• Strong communication and documentation skills.
• Must be authorized to work in the United States.
• Must have updated medical and vaccination records or be willing to obtain them.
• Must complete the required drug screening or habituation form.
• Must successfully complete Alvita Care's onboarding process before starting the assignment.
• Must be able to meet the facility's scheduling and staffing requirements.
