Job Summary
In addition to their regular daily shift LPN duties, the Lead LPN will perform additional clinical and administrative tasks that will provide support to their clinical team and practice. The Lead LPN will be responsible for recording patient's vital signs, patient intake and output, and recording patient history. This associate will administer medication, injections and functions in scope of license. This position will clean equipment in patient care areas and ensure patient care rooms are kept clean and supplies are well stocked. The Lead LPN will maintain records of patient activities and observations and submit records to the appropriate clinical staff regarding patient's condition. The Lead LPN will assist with scheduling patients and authorizations to support administrative staff as needed. The Lead LPN will train and monitor all clinical associates on all related clinical and clerical workflows. The Lead LPN will advise the Practice Manager of associate performance to address any areas of training and development required.   The Lead LPN will be responsible for day-to-day activities including, but not limited to, planning and development, scheduling and daily operations.  The Lead LPN will ensure their clinical team works with clinical and clerical support staff to deliver excellent patient care and promote patient satisfaction. The Lead LPN will also practice under the direction of the Practice Manager.

Essential Functions

  • 1. Administers medications, injections, insulin, immunizations, perform finger sticks and functions in scope of license. ADA
  • 2. Identifies patient goals, communicates, and collaborates with clinical team to support patient care plan. ADA
  • 3. Performs clinical workflows. ADA
  • 4. Monitor and record patient's vital signs, patient history and reason for visi.t ADA
  • 5. Explain treatments and procedures to patients and families. ADA
  • 6. Assist with scheduling, referral and follow up appointments. ADA
  • 7. Cross cover multiple practices. ADA
  • 8. Assist with authorizations and administrative needs as needed. ADA


Qualifications

  • High School Required
  • AS Preferred
  • 4-6 years 1-5 Years (Tier dependent) Required
  • Annual Assessment - OHS Required
  • Licensed Practical Nurse New York - New York State Board of Nursing Required
  • National Council Licensure Examination for Practical Nurses (NCLEX-PN) or another license examination acceptable to NYSED Required
