Home Health Aide (HHA)

SUMMARY:

Upon orders of the physician and under direct supervision of the Clinical Supervisor, provides personal, environmental, and nutritional services for patients in their place of residence.

DUTIES AND RESPONSIBILITIES:

• Following the HHA plan of care, assists with/attends to patient’ s ADLs including but not limited to bathing and/or assisting with oral hygiene and skin care; shampooing; changing bed linens; assisting with ambulation and prescribed exercises; light housekeeping, meal prep, and laundry.

• Turns and positions bed patients.

• Provides teaching and emotional support to patients and families.

• Observes patient’ s physical and emotional status; takes action in emergency situations.

• Reports all observed or stated patient changes to clinical supervisor.

• Prepares timely, legible, and complete documentation of all care provided; returns documentation to office/department according to established policy.

• Documents all communication with patients.

• Participates in care coordination meetings.

• Recognizes limitations in providing care and performs prescribed duties as allowed by law.

• Participates in continuing education to continually improve skills and abilities and stay abreast of current technologies/practices.

• Participates in the QAPI information collection process.

• Exhibits a high degree of courtesy, tact, and poise when interacting with patients, families, and other healthcare professionals.

• Adjusts to fluctuating peaks in patient flow, acuity, and other operational demands while maintaining quality.

• Performs other related duties as assigned by management.
