About the role

  • High school diploma or equivalent required.
  • One year of caregiving, personal care, or home care experience preferred.
  • Previous experience in home health, hospice, assisted living, or long-term care settings preferred.
  • Active Connecticut Home Care Aide Certificate and/or Connecticut CNA certification preferred.
  • Valid Connecticut driver's license and reliable transportation required.
  • Strong interpersonal and communication skills.
  • Ability to provide compassionate care to patients experiencing chronic illness, advanced disease, or end-of-life care.
  • Experience working with seniors and supporting patients and families through hospice services preferred.
  • Ability to work independently while remaining part of a collaborative healthcare team.
  • Familiarity with electronic documentation systems preferred.
  • Fluent English required; bilingual candidates are encouraged to apply.
  • Flexible availability, including part-time schedules of approximately 20 hours per week, with the opportunity for additional hours based on patient needs.

The Hospice Aide/Home Care Aide provides compassionate personal care and assistance with activities of daily living to patients in their homes, assisted living communities, skilled nursing facilities, and other community-based settings. Working under the direction of the Clinical Case Manager, this role supports patients in maintaining comfort, dignity, independence, and quality of life while following the established plan of care. The ideal candidate is caring, dependable, and committed to delivering exceptional support to patients and their families.

Responsibilities

  • Assist patients with activities of daily living, including bathing, dressing, grooming, toileting, and mobility assistance.
  • Provide emotional support, companionship, and comfort to patients and their families.
  • Document services and patient interactions accurately in the Electronic Medical Record (EMR).
  • Assist with routine range of motion exercises as directed by therapy staff.
  • Remind patients to take medications as prescribed and assist with medication self-administration when appropriate.
  • Prepare meals and encourage proper nutrition and hydration.
  • Perform light housekeeping and home management tasks to support a safe and comfortable environment.
  • Observe, monitor, and report changes in a patient's physical, emotional, or mental condition to the clinical team.
  • Participate in required trainings, staff meetings, case conferences, and quality improvement initiatives.
  • Follow all agency policies, safety standards, and confidentiality requirements.
  • Perform other duties as assigned.
