Horizon Home Health & Hospice is seeking compassionate, dependable, and patient-focused Certified Nursing Assistants (CNAs) to join our growing team serving patients throughout the Twin Falls and Burley communities. As a CNA, you will play a vital role in helping patients maintain comfort, dignity, independence, and quality of life in the place they call home.

Position Summary

The Certified Nursing Assistant provides personal care services and supportive assistance to patients in accordance with the patient's plan of care and under the supervision of licensed clinical staff. This position requires excellent communication skills, compassion, reliability, and a commitment to delivering high-quality patient care.

Responsibilities

  • Assist patients with activities of daily living (ADLs), including: BathingDressingGroomingToiletingAmbulation and transfers
  • Provide companionship and emotional support
  • Assist with light housekeeping related to patient care
  • Monitor and report changes in patient condition to the nursing team
  • Document services provided according to agency policies
  • Follow infection control, safety, and confidentiality protocols
  • Support patients and families with professionalism, respect, and compassion
  • Travel to patient homes within the assigned service area

Qualifications

Required:

  • Current Idaho Certified Nursing Assistant (CNA) certification
  • Active CPR certification or ability to obtain upon hire
  • Valid driver's license and reliable transportation
  • Ability to pass background screening and drug testing
  • Strong interpersonal and communication skills
  • Ability to work independently while following a prescribed plan of care

Preferred:

  • Previous home health, hospice, long-term care, or healthcare experience
  • Experience working with geriatric and medically complex patients
