TITLE:  Field Nursing Supervisor

Reports To:  Director of Patient Services

Salary: $89,000-$104,000/ yr

 Location: Field. Position is primarily in patient homes within the assigned service area (Westchester County, Putnam County, and New York City), with periodic time in the branch office for team meetings, documentation, and in-services. Travel between assigned patients is required throughout the workday.

The Field Nursing Supervisor provides clinical oversight and supervisory visits for caregivers in patient homes, ensuring care is delivered safely, competently, and in compliance with NYSDOH regulations and the patient’s Plan of Care. This role serves as a key clinical resource for patients, families, and field staff.

ESSENTIAL JOB DUTIES:

  • Conduct required in-home supervisory visits per NYSDOH regulations and agency policy.

  • Assess patient needs, environment, and Plan of Care implementation during supervisory visits.

  • Evaluate Home Health Aide and personal care worker performance and competency in the home.

  • Identify and address clinical concerns, care gaps, and safety issues; communicate findings to the care team.

  • Provide on-the-spot coaching and training to field staff during visits.

  • Document supervisory visits accurately and timely in the EMR.

  • Respond to clinical questions and incidents from field staff, patients, and families.

  • Collaborate with intake, scheduling, and operations on case starts, transitions, and care plan updates.

  • Participate in on-call rotation as scheduled.

  • Support the agency’s PI/CQI program through chart audits and process improvement activities.

  • Other duties as assigned by leadership.

AGE RELATED SKILLS & COMPETENCIES:

  • Oversees care delivered to geriatric, adult, and (where applicable) pediatric patient populations.

  • Ensures field staff competency in age-appropriate care techniques and communication.

  • Models culturally sensitive, age-appropriate interactions with patients, families, and staff.

QUALIFICATIONS:

  • Active NY RN license in good standing required.

  • Associate’s degree in Nursing required; BSN preferred.

  • Minimum 2–3 years of nursing experience required; home care and supervisory experience strongly preferred.

  • Current CPR/BLS certification.

  • Strong knowledge of NYSDOH home care regulations and supervisory visit requirements.

  • Excellent assessment, coaching, and communication skills.

  • Proficiency with electronic medical records; HHAeXchange preferred.

  • Reliable transportation, valid NY driver’s license, and proof of auto insurance.

PHYSICAL ACTIVITIES:

  • Frequent standing, walking, bending, stooping, kneeling, and reaching during patient visits.

  • Lifting and carrying nursing supplies and equipment up to 25 pounds; occasional assistance with patient transfers up to 50 pounds.

  • Manual dexterity required to perform clinical procedures, administer medications, and operate medical equipment.

  • Ability to drive between patient homes and to and from the office throughout the workday.

  • Climbing stairs may be required to access patient homes.

  • Prolonged periods of standing, sitting, and computer use for documentation.
