Job Summary: Practices principles of Person Centered Care in the performance of primary job function.
Within a restorative framework, provides basic nursing and personal care for residents through the RNO. Assists residents with hygiene, mobility, toileting, eating, and participating in recreational activities. Works to maintain a clean, safe, homelike atmosphere, and respects the individuality of each resident.

· Assists with move-ins by welcoming new resident and family, by providing for basic immediate needs, and by gradually introducing resident to other residents and scheduled activities.
· Assesses resident needs, and provides necessary nursing care as per resident's care plan.
· Handles resident, family, or physician complaints/concerns, referring issues to DON as necessary.
· Respects advance directives and assists residents to a dignified death; and provides emotional and spiritual support to family and other staff as needed.
· Responds to life threatening situations based upon nursing standards, policies, procedures, and protocol.
· Participates in department and/or unit performance improvement activities.
· Conducts rounds with oncoming CNAs to assure continuity of care to residents.
· Provides direct physical care (bathing, grooming, feeding, toileting, transferring/ambulating) to residents to the extent the resident is incapable of performing those activities.
· Monitors residents and report all new signs, symptoms, resident complaints, alterations in skin integrity, and changes in functional status to licensed nurse or nursing supervisor.
· Assists with admissions to Assisted Living by preparing room, welcoming new resident and family, by providing for basic immediate needs, and by gradually introducing resident to schedule and activities for residents.
· Answers call lights promptly which allow residents to know if and how long a delay to expect; notifies charge nurse or nursing supervisor if he/she cannot complete work in a timely fashion.
· Transports residents to and from appointments and assists Life Enrichment personnel in transporting to and from activities.
· Resolves problems/interpersonal issues in a professional manner or requests assistance from supervisory personnel.
Use equipment as required in care of residents (i.e. reads digital thermometer, balances and reads
· scale, reads graduated cylinders, whirlpool tub, shower and tub chairs).
· Documenting Resident Care on COW (Computer on Wheels) i.e. vision, etc.
· Supports a restorative philosophy of care.
· Works to maintain a clean, safe, homelike atmosphere, and respects the individuality of each resident.
· Recommends, provides, and participates in meaningful therapeutic activities suitable to resident needs, abilities and interests.
· Maintain the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information.
· Comply with applicable federal, state and local laws, rules, and regulations; maintains knowledge of and follows all Plymouth Place policies and procedures.
· Maintain regular and punctual attendance at work and meetings.
· Attends in-service and education programs.

JOB REQUIREMENTS:
· Successful completion of a State approved CNA course. Successful CNA competency testing.
· Basic Life Support (CPR)
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
· Knowledge of charting systems( Point Click Care)
· Familiar with patient care
· Proficient with Microsoft Office Suite or related software.
· Solid organizational skills including attention to detail and multi-tasking skills.
· Ability to read, write and speak English sufficient to perform job duties and to interact and communicate with residents, families, and employees.
ENVIRONMENTAL AND PHYSICAL ACTIVITY REQUIREMENTS: The physical activities of this position constantly require effective talking, hearing, sitting, walking, and standing functions. The position also is often requires exerting up to 50 pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Must be able to perform activities with close visual acuity, such as reading a computer screen and reading print material. The functions of this position are conducted in a temperature-controlled environment.
OSHA Exposure Category: Category I – Position includes tasks that involve exposure to blood, body fluids, tissues; would be required to perform Category I tasks.
